You Cannot Fix It Until You Know Which Layer Broke.
Infrastructure decides whether the message arrives. Data decides whether you reached the right person. Targeting decides whether they have a reason to care. Message decides whether they reply. These fail independently and produce similar-looking symptoms, so guessing which one is broken and rewriting copy has roughly a one-in-four chance of addressing the actual cause.
Infrastructure failures look exactly like message failures from the reply-rate dashboard.
Bad data produces low reply rates that no amount of copy work will improve, because the recipient was never the right person.
Targeting failures are the most common and the least examined. The message was fine and the recipient had no live problem.
Diagnose the Layers in Order
Layer One: Is It Arriving?
Seed placement tests, blocklist checks, authentication audit and bounce classification. If placement is degraded, every other measurement you take is meaningless because your denominator is wrong. This is a day of work and it eliminates or confirms the most common cause.
Layer Two: Is the Data Right?
Verify a sample of contacts manually: is the person still there, is the title accurate, is the company still what your data says. Poor data quality is invisible in aggregate metrics and it is a frequent cause, particularly for teams targeting markets their enrichment vendor covers badly.
Layer Three: Do They Have a Reason to Care?
Examine what triggered contact with each account. If the answer is that they matched a firmographic filter, that is the problem. Companies matching an industry and headcount range have no particular reason to respond this week, and this is the most common failure by a wide margin.
Layer Four: Only Now, the Message
If mail arrives, data is accurate and targeting is trigger-based, then message quality is genuinely the variable. Test the opening and the specificity of the evidence rather than the subject line. The first sentence carries far more weight than most teams' testing priorities suggest.

Common Questions
Is outbound dead?
Undifferentiated volume outbound is, and its decline is well documented. Precise, trigger-based outbound to a small number of accounts with a live reason to care continues to work, and works better than it did, because the noise from everyone else's volume makes a genuinely relevant message stand out more.
How long should we test before concluding it does not work?
A full cycle: three to four weeks of warm-up, then at least six weeks of sending at meaningful volume with proper measurement. Teams that conclude at three weeks are usually measuring their warm-up period, which tells them nothing about the motion.
Should we hire an SDR or fix the system?
Fix the system first. An SDR added to broken infrastructure produces the same poor results at higher cost, and it is a demoralising first job for whoever you hire. Once the layers are working, an SDR has something to scale rather than something to compensate for.
What reply rate should we expect from a working system?
Meaningfully higher than volume outbound, at much lower send counts, but the number varies too much by market and offer to promise. The more useful measure is pipeline per thousand sends, which is what actually determines whether the motion is worth running.
